乙方:_

經(jīng)甲乙雙方友好協(xié)商，現(xiàn)就甲方委托給乙方完成的_申報(bào)中文材料翻譯成英文材料事項(xiàng)簽訂如下協(xié)議。

一、翻譯稿件名稱：_材料。具體包括：

1、擬建_考察報(bào)告(含建設(shè)發(fā)展規(guī)劃及規(guī)劃圖冊(cè));

2、_申報(bào)書;

3、_申報(bào)自評(píng)報(bào)告;

4、_風(fēng)光片解說詞。

二、工作時(shí)間：甲方于x年x月日前將需翻譯的中文定稿材料交付乙方。乙方應(yīng)于x年x月x日前將翻譯好的英文成稿交付甲方。

三、交稿方式：乙方應(yīng)向甲方提供英文成稿打印件及電子文本(文件格式：coredraw9.0排版)各一份。

四、協(xié)議總金額：協(xié)議全部工作任務(wù)總費(fèi)用為_元，大寫人民幣_(tái)元整。甲方在簽訂協(xié)議之日起向乙方支付x萬元，余款在乙方交付成稿并經(jīng)甲方驗(yàn)收后一次性結(jié)清。

五、翻譯質(zhì)量：乙方保證翻譯成稿質(zhì)量，做到忠實(shí)原文、翻譯準(zhǔn)確、語句通順、行文流暢，達(dá)到甲方提供給乙方的《_綜合報(bào)告》(英文版)的翻譯水平。如雙方對(duì)譯文水平發(fā)生爭議，由雙方共同認(rèn)可的第三方進(jìn)行評(píng)判。

六、其它事項(xiàng)：乙方負(fù)責(zé)為甲方在_申報(bào)國際評(píng)審會(huì)上作英文陳述，陳述費(fèi)用不再另付。乙方在申報(bào)材料的英文翻譯稿進(jìn)行電腦排版時(shí)，需就排版格式等有關(guān)問題與甲方提供的印刷廠技術(shù)員進(jìn)行聯(lián)系溝通，以保證英文成稿的電子文本符合甲方印刷要求。

七、本協(xié)議自簽訂之日起具有法律效力，雙方應(yīng)共同遵守，否則由違約方賠償對(duì)方由此造成的一切損失。

八、本協(xié)議未盡事宜，由雙方友好協(xié)商解決。

九、本協(xié)議壹式肆份，甲乙雙方各執(zhí)貳份，具有同等法律效力。

十、病人外出，幫其安排汽車；

十一、向病人發(fā)放follow-upinformation；

十二、跟蹤病人的康復(fù)評(píng)估。

除了上述工作之外，我還協(xié)助同事完成其他一些工作，如：換錢、打印細(xì)胞通知單，打印病人康復(fù)計(jì)劃表，翻譯病人博客，幫病人更改機(jī)票等。

對(duì)于以上工作，我都能積極主動(dòng)并及時(shí)的完成。有時(shí)候由于各種原因，可能出點(diǎn)差錯(cuò)，但我能夠及時(shí)的糾正。能夠很好的和本部門以及其他部門的同事合作，有時(shí)可能會(huì)由于工作原因產(chǎn)生一點(diǎn)分歧，但能夠積極有效的與他們進(jìn)行溝通。生活和工作中可能有沒有注意或被疏忽的小問題，以后我會(huì)“靜坐常思己過”，及時(shí)把問題解決并把問題的不良影響降到最低。
